Assignment Title

The model of nursing i will use will be the Roper,logan and tieney 12 activities of daily living ,these will be explained in the nursing process.

The patient I have chosen is a 38 yr old married man called Mr P his identity has been protected for confidentiality purposes and informed consent ha been given to use his nursing notes. he has two children a boy and a girl. He works as a joiner and has done since he was 16. He had been having spells of dizziness,shaking and loss of power of his lower limbs and right arm. This has made him feel depressed and question his self worth. He’s attended his GP and told him of his symptoms his GP referred him to neurologist and he has had tests carried out. He has been asked to attend an appointment where he will be diagnosed with Parkinson disease. Upon arrival to the clinic he tells me he is extremely anxious about the appointment as he has a fair idea what he is here for and that his diagnoses is PArkinsons disease.

· The rationale for your choice of patient/ client · Briefly describe the Nursing Process · Outline the identified Model or framework of nursing used · Discuss the nature and purpose of the nursing assessment on your chosen patient/ client.

· Describe the skills and tools used in this assessment. (Communication, dignity, respect etc. should be evident in your description.) · Outline the findings of your assessment on your chosen patient/ client.

· From the patient/ client assessment clearly state one key patient/ client problem.

· All literature used in this assignment should be referenced using the Harvard system the word count should be 2000 with the allowance of 200 words over no more.
