Case:
Luis Acosta, a safety engineer at Velez Manufacturing, was walking through the plant when he encountered a situation that caught his attention. Some employees emerged from a room where various chemicals were used in the manufacturing process. Acosta inspected the room but was able to determine that something was wrong or even different from any other day, but what was worrying is that the workers were refusing to resume their tasks. They refused to work and insisted that the conditions in the room were unsanitary. Acosta and the group supervisor analyzed the situation and wondered if they should order the staff to resume their work since production was delayed.
