A summary and signature sheet

Identify the payment criteria to determine whether payment should be made

Attach the scope baseline, schedule, and budget. Include the original documents from early in the class, with all change requests.

I have already completed the bottom portion. All I need is the Summary sheet and the signature sheet. Payment criteria portion is up to the writer.

Please assign a writer that is proficient in project management forms. An outside template can be used if necessary. Please cite the source from where you obtained the template.
