  • Your report, including any other attachment must both be submitted as single files. Do not split your report, bibliography or appendix into separate documents.
  • If you have hand written work such as calculations or sketches, these can be scanned and incorporated in a PDF for submission.

The Brief

Cabin crew and passengers remain at the heart of aviation safety, as such, the need for careful consideration of Human Factors continues to increase. The Aviation Industry requires a strong commitment to and understanding of  human performance issues  in addition to engineering excellence to meet the safety performance and business requirements of all stakeholders. Therefore, as an aerospace engineer, it is expedient to have an in-depth understanding of the aerospace industry requirements to ensure safety, efficient performance, and cost reduction through the application of Human Factors in Aviation skills and knowledge.

In order to carry out this assignment, you first need to conduct a thorough research into Human Factors related issues in regards to the aerospace industry. This report will assess your application of knowledge and skills within the aerospace industry, your personal reflections, as well as your ability to relate and translate theoretical concepts into a real life situation. Select any ONE of the following titles for your research and coursework.

 

1 Critically evaluate the impact of Human Factors within a series of aviation/aerospace/space related incidents with particular reference to the concept of the ‘dirty dozen’ as identified by ‘Gordon Dupont, (Maintenance And Ramp Safety Society (MARSS) Canada 1993).
2 Examine at least four case studies provided by aircraft accident investigation organisations and explain how Reason’s (1990) ‘Swiss Cheese model’ can be applied to identify the main contributory factor(s) in causing safety critical incidents. Critically examine the shortcomings of Reason’s original concept and make suggestions how the model could be (and has been) developed further.
3 As space travel is opened up to commercial operations, critically examine the impact that  Human factors concepts may have  on profit making organisations, using case studies to illustrate some of the challenges that they might face.
4 Using recognised Human Factor models, critically analyse at least four case studies to illustrate how maintenance errors have influenced the flight safety of aircraft and contributed to the  development of  the concept of a ‘no blame’ culture.
5 Critically examine the work of Air Accident Investigation reports from at least two major sources, using at least four case studies and analyse the way that they have influenced the development of Crew resource management (CRM) since 1945.
6 Critically examine and evaluate the safety record of at least two different aircraft types and explore the way the design features, combined with Human factor issues have played a major impact on safety critical incidents.
7 Critically describe and analyse the ways in which Human Factors related models can improve Aircraft Maintenance safety, providing examples from at least five researched case studies.
8 In what ways have the move to more automated flight systems impacted on Human Factors and CRM in new generation passenger aircraft?  Provide at least two case studies to illustrate your critical analysis.
9 Critically analyse challenges associated with the practical application of the currently available safety tools associated with Human Factor models, exploring what can be identified as current strategies to enhance understanding and management of maintenance errors.
10 Compare and contrast the way in which The TEM (Threat Error Management) model involving Latent and Active errors contribute to aviation incidents that have been formally reported during the past fifteen years. Using case studies, critically analyse the way in which such incidents could have been mitigated by Threat Error Management.
