Visual Infographics present information, data, or knowledge intended to quickly and clearly communicate information to an audience.
You are to research a chronic and/or complex health issue that interests you and develop an infographic, suitable for social media, that uses best available evidence to communicate information about health promotion or primary care and self-management for people and their families living with the chronic and/or complex health issue.
You will be required to submit ONE PDF document via URKUND and Canvas containing:
· Infographic suitable for social media as a PDF (1 page)
· Summary of information (1000 words, +/- 10%) used to inform the details in your Infographic in relation to health promotion or primary care and self-management, including early detection and secondary management strategies.
· Reference list containing high quality, peer reviewed, current (less than 10 years) literature and credible professional websites.
Infographic:
You first step is to research and collate information about a chronic and/or complex health issue that interests you. Ensure you are using up to date, reliable and relevant sources. From that information identify the key points you would like to make in the infographic.
The next step is to create the Infographic usingCanva (Links to an external site.). The free version is sufficient for this assessment.
Once you have accessed the online program, search for Infographic. You can choose one of many templates available or create your own. Have fun and get creative.
Review the relevant areas of the rubric to ensure you are meeting the assessment requirements. Sources of information do not need to be cited on the Infographic as you will be providing them in your Summary.
Summary of Information:
The Summary of information (1000 words, +/- 10%) includes intext citations and must demonstrate a fluent professional/academic writing style within the word count.
The Summary of information provides the supporting evidence for details and expands on the brief information within your Infographic.
The reference list is accurate using APA 7thEd referencing system and contains high quality, peer reviewed, current (less than 10 years) literature. Use of credible professional websites is acceptable, but please ensure their quality, relevance, and currency of information.
As with all academic assessments, students are required to comply with the University’s policy and procedure for assessment items, inclusive of ensuring academic integrity of their work. The Assessment will be addressing learning outcomes:
1. Articulate health promotion or primary care in the context of chronic and complex care;
2. Identify key self-management principles and strategies
3. Demonstrate understanding of intensive (focused) care coordination for people with complex, unstable and comorbid conditions during hospitalisation, including early detection and secondary prevention strategies.

2) Identify key self-management principles and strategies, early detection and secondary prevention strategies for people and their families living with the chronic and/or complex health issue. Provides a sophisticated, thoroughly considered, and comprehensive response that clearly identifies and includes key self-management principles and strategies, early detection and secondary prevention strategies within the Infographic relating to the chosen health issue
3) The flow of ideas is structured visually to ensure the Infographic is organised and logical for readability and engagement.
