Each group will present a 5-6 minute speech presentation depicting Integrated Care using TeamStepps and TeamStepps “I PASS the BATON” used in transitions in care. A minimum of 5 peer-reviewed articles must be used to support the analysis of the group’s needs assessment and action and evaluation. The speech needs to include:
- An accurate representation of the need at Kahi Mohala Behavioral Health. The topic area needs to be clearly understood in the speech presentation
- The speech must show creativity and accurateness of information. The speech must clearly represent the following: the needs, plan of action, and evaluation

After team review of a needs assessment, we are all in agreement to complete a final presentation and video about TeamStepps in Integrated Care. Our goal is to transition from tape recording shift reports to live shift reporting using the handoff design “I Pass the Baton”. Patient care would be seamless, as all vital issues would be directly communicated. Preparation of the next shift will also reinforce a team approach from one shift to the next. Patients will feel that there is no drop off in interest or intervention and this would engender greater trust in the therapeutic process.
Needs Assessment: (Create a SWOT Analysis)

- Live shift reports directly relay vital issues
- Live shift reports reinforce a team approach seamlessly
- Live shift reports will reduce/eliminate nurse “talking over” while the tape recording is being played
- Live (face-to-face) shift report is best evidence based practice
- Using TeamSTEPPS is a great way to implement changes
- TeamSTEPPS may be used to incorporate inconsistencies in shift report
- According to Kahi Mohala nurse manager, communication is one of the biggest challenges. She is also very interested in implementing TeamSTEPPS

Improving Shift Reports
- A Quality Improvement project was conducted at Kahi Mohala in Ewa Beach, Hawaii to implement best practices in patient handovers at change of shift in the Inpatient Psychiatric Program
- The Quality Improvement project was initiated February 10, 2015 and was conducted by five Chaminade University of Honolulu nursing students

- A nursing shift reports worksheet was developed using the I PASS the BATON handoff template to standardize communication of technical information about patients relevant to their condition and care at change of shift
- The handoff template was then used by the nursing students to compare the similarities and differences of Kahi Mohala’s inpatient end of shift report to a live shift report using the handoff template created
