A medical–surgical unit at your hospital wants to change from using a team nursing model to a primary care nursing model. The unit director asks the group to develop a logic model tool to show the problem, goal, resources, activities, and short-term goals involved in making this type of change. You are one of the staff members charged with preparing these aspects for the logic model. Discuss the details of the logic model you prepared. Items to consider when completing this request include the following: The initial step in this process of establishing a logic model is the analysis of the challenge identified or the problem. Once the problem is determined clearly, a goal for the project can be worded. The problem statements can also be classified as issue statements or situations. Usually the problem statement addresses the ideas of who, what, why, where, when, and how. The problem statement and goal should be narrow in focus, while objectives used within the process can reflect the scope of the project. From the activities, the statement is depicted: If the activities are successful, then certain outputs such as products or services will be completed. These outputs should be measurable, perceptible, and ensuing from the activities. Once the outputs are determined, the outcome declaration demonstrates the following: If the deliverables are obtained, then the participants will be rewarded with certain benefits. The outcomes are the results, impacts, and/or objectives for the project. Outcomes should be measurable and reflect the change that occurs from the project process. Outcomes can be viewed as short-term (1–3 years).
