Assignment Details
The aim of this assignment is to enable the students to assimilate the fundamental principles underlying Safety Case Development and Management and to apply these to industrial applications (in this case Coast Guard Helicopter). The assignment is to enable students to apply theoretical and practical themes introduced in previous years to develop safety-related problem solving skills. The aim is to further improve understanding of Hazard Identification risk analysis and management etc and develop skills in their quantification and analysis.Safety Case for Coast Guard Helicopter, the following information is provided for use in the assignment:
CGH Concept of Operations
CGH Roles
- Respond to emergency calls, Search and locate persons & vessels in distress (off-shore and in-shore), Provide first responder medical facility on ground and onboard CGH, Recover casualties and transport to hospital.
CGH operating environment:
- Coastal waters and in-shore
- Extreme weather conditions
Personnel
- Crew: Pilot, Copilot, Flight Mechanic and Rescuer, Casualties: Up-to 4 passengers: 2 stretchered, 2 seated
Performance
- Maximum speed: 300km/h, Range: 600km, Service ceiling: 5,000m
CGH Operations
- Continuous communication with Coast Guard HQ, Take-off, Transit to patrol area, Search & locate persons / vessels in distress, Deploy rescuer personnel, Deliver first responder medical care, Recover rescuer and casualty personnel, Transit to destination, Deliver casualties to hospital, Training, Reporting and Maintenance.
CGH Functions

CGH Navigation
- CGH has two separate Navigation systems for determining CGH current position:
- GPS
- Position is calculated via the civil GPS system
- Normally very accurate provided satellite link is available
- On-board inertial navigation
- Integrates compass direction and vehicle speed over time to calculate direction and distance from a known point giving location
- The known point needs to be updated regularly by taking a “fix” – the inherent calculation errors cause an average of 100m per hour drift between actual and calculated position
- Charts
- The calculated position is overlayed onto electronic charts similar to SatNav systems
- The co-pilot also maintains a cross check using paper charts
Task – you are required to write a Safety Case for Coast Guard Helicopter, this must include a Full high-level Safety Case Structure, Partially populated with argument and evidence, refer to Scale Model + Photographs for visualisation
- Legislation
- List 5 items of applicable legislation
- Present sample evidence for compliance with one of these (minimum 5 clauses)
- Hazard Identification
- Include sample evidence from each of the following activities:
- HAZOP study of the deploy rescuer function
- A Zonal Hazard assessment
- A functional failure assessment of the Safe Navigation function (you will need to decompose the function)
- Hazard Analysis
- For 2 hazards identified provide sample risk assessment
- State the risk reduction measures for each and include at least one measure of each of the following types:
- Operating practice
- Engineered Barrier
- Provide sample evidence for the integrity of each of the barriers
- Provide an ALARP statement for each of the above hazards including reference to requisite evidence
