Depression is said to be the fourth leading contributor to the global burden of disease (WHO 2012). Locally, the Australian Institute of Health and Welfare (AIHW 2012a, p. 117) reports mental ill health as the ‘leading cause’ of nonfatal disease and injury, accounting for an estimated 24 per cent of the total years lost due to disability (AIHW 20llb, p. 1).
