BP 145/90. HR 105, RR 25 SaO2 99%,
D – severe abdominal pain 8/10 O – Intermittent pain over the last few weeks but became severe 2 hours after dinner, L – upper abdominal pain O – Nausea and vomiting, occasional constipation R – initially relieved after vomiting but this time unrelieved.
Abdomen tense, appears distended.
